Dermal fillers assist in decreasing facial lines and return volume and fullness to the face. With age, several changes occur. There is a loss of facial volume as the subcutaneous fat decreases, and the skin has increased laxity. As there is less fat, the facial muscles work closer to the skin, thus making wrinkles more noticeable. Lastly, skin changes manifest by fine lines, wrinkles and blood vessels becoming more prominent. Factors such as heredity, sun exposure and lifestyle also affect the facial skin.
What dermal fillers can do:
Dermal fillers can be used alone to treat early facial ageing or combined with facial rejuvenation surgery as a value add-on.
Dermal fillers can only take so far on the anti-ageing highway. Sooner or later, only surgery such as a facelift, browlift or blepharoplasty will be able to rejuvenate the aged face. Minimally invasive treatments using soft tissue fillers have their place and role to play. They can delay facial rejuvenation surgery, but not indefinitely. Eventually, one will need a “tincture of cold steel”. Dermal fillers are only temporary treatments for facial ageing, and that ongoing treatment will be required for long-term results.
Before the procedure, the face is cleaned, and a local anaesthetic cream is applied. Some fillers contain a local anaesthetic agent. The Filler is injected directly into the area in question with a thin needle or cannula. After the procedure, there may be some temporary swelling, redness or minor bruising at the injection site. An ice pack can be applied to reduce swelling and alleviate discomfort. Although the area may feel a bit tender for a day or two, it is usually not painful enough to require any medication. This usually subsides quickly and makeup can be applied immediately after the treatment.
Precision Aesthetics has a variety of Tissue Filler substances available that can address a wide range of cosmetic flaws to fulfil the needs of each patient. Each of our Fillers can offer subtle, distinctive benefits and more than one Filler may be used to achieve the best results. Some Fillers may provide longer-lasting results while others allow for more precision, with fine lines gradually bringing the face into better balance, creating an overall rejuvenated appearance
1. Calcium hydroxylapatite
Brand names include: Radiesse
Calcium hydroxylapatite is a mineral-like compound and is found naturally in human bones. It’s often used to:
Calcium hydroxylapatite is produced biosynthetically i.e. no animal products or animals are used. This requires no skin testing and lowers your risk of an allergic reaction This dermal Filler produces a very natural result, doesn’t migrate, and has rare side effects. This dermal Filler was used first in dentistry and reconstructive surgery and has a long safety record.
2. Hyaluronic acid
Brand names include: Juvederm, Perlane, Restylane, HAmonyCa
Hyaluronic acid injections are used to improve the skin’s contour and reduce depressions in the skin due to scars, injury or lines. You can see significant enhancements for:
Hyaluronic acid is found naturally in your body. There are high concentrations found in soft connective tissues and the fluid surrounding your eyes. It’s also in skin tissue, some cartilage and joint fluids. It is removed and reformulated and now has become one of the most popular kinds of injectable fillers.
3. Polylactic acid
Brand names include: Sculptra
Polylactic acid (PLLA) is a synthetic dermal filler injected into your face, causing your body to produce collagen. This dermal Filler type is known as a biostimulator. It is a non-toxic, biodegradable substance that has been used for over 40 years as a suture material.
PLLA is known to work very well in the lower half of your face, and is used to:
This substance unlike other dermal fillers doesn’t produce immediate results. Instead, it stimulates your body’s own collagen production, so results appear gradually over the next few months.
You will need 3 treatments to achieve your desired results. There is re-stimulation of your collagen with each treatment, and it can take up to four to six weeks to see the full result. Although this dermal Filler type is considered semi-permanent, you may still need occasional touch-ups.
Dermal fillers differ in chemical makeup, and longevity having varying degrees of softness. Thicker fillers are for foundation and injected deep onto the bone.
Dermal fillers generally have minimal risks and side effects with predictable results and are often injected in the surgeon’s office.
Results from soft tissue augmentation using dermal fillers are apparent immediately. They will last from several months to several years depending on the filler used and the patient injected. Maintaining good overall health and healthy and strong skin with appropriate products and treatments will improve the results and longevity.
Dermal fillers are should only be injected when necessary to restore and maintain balance, symmetry, proportion and even transition zones to the face i.e. a smooth and youthful shape. Although some may last for two years, others must be repeated every 8-12 months for best effects. It is important to avoid overfilling or a “pillow face” result.
Avoid topical products such as Tretinoin (Retin-A), Retinol, Retinoids, Glycolic Acid, or any “anti-aging” products. Avoid waxing, bleaching, tweezing, or using hair removal cream on the area to be treated.
Generally, the effects of the HA fillers will wear off after a few months, Biostimulators will last longer. Eventually the treated area is likely to return to its original state. The benefits of fillers will be gone and you’ll notice that those lines and volume loss will come back.
At Precision Aesthetics in Johannesburg, we use the VECTRA® 3-D imaging system during consultations to provide our patients with what we believe is the most complete education possible regarding their cosmetic and plastic surgery procedures. It also helps to demonstrate to them the results they can expect.
